The community health needs assessment (CHNA) process is done in collaboration with a variety of community partners and seeks to address the health needs of all community members, in particular those that are underserved, with high needs and limited resources.
Federal regulations require that all non-profit hospitals assess the health needs of communities they serve and develop a plan to address those needs every three years. Our findings help us understand your community's overall health. This allows us to improve our services and adjust our community health plans.
