How to Qualify for Metabolic & Bariatric (Weight Loss) Surgery

​​In order to qualify for metabolic and bariatric surgery, you need to meet specific criteria. Your insurance may require additional criteria to qualify.

To qualify you need a BMI greater than or equal to 40 or BMI greater than or equal to 35 with a co-morbidity. Co-morbidity conditions include, but are not limited to:

  • Anxiety
  • Arthritis
  • Asthma
  • Coronary artery disease
  • Cardiomyopathy
  • Deconditioned
  • Depression
  • Dyslipidemia
  • Gallbladder disease
  • GERD
  • Heart failure
  • Hernia
  • Hypertension
  • Hypoventilation syndrome
  • Infertility
  • Kidney disease
  • Liver disease
  • Mechanical arthropathy
  • Migraine
  • Pickwickian syndrome
  • ​​PCOS
  • Poor healing
  • Pseudotumor cerebri
  • Pulmonary hypertension
  • Restless leg
  • Sleep apnea
  • Type 2 diabetes
  • Urinary incontinence
  • Venous stasis
  • Many others

You must also:

  • Have been unsuccessful with serious attempts at weight loss.
  • Understand and accept the risks of metabolic and bariatric surgery.
  • Be prepared to make permanent changes in your diet, activity level and lifestyle.
  • Agree to adhere to life-long follow-up visits with your surgeon or health care team to monitor your health status.
