Knowing your health insurance coverage is very important. Most often your insurance coverage questions can be answered by your insurance company.
If your insurance plan is not on
this list, check your benefits handbook, employer or insurance company to learn about your coverage. For questions about coverage, contact your insurance company before a Clinic visit.

Referral and prior authorization
Know your insurance plan requirements for referrals and if you are responsible for payment, or your appointment may be rescheduled.
Many insurance plans require prior approval for care. On request, our
Patient Assistance Staff can help you get prior approval. While insurance may approve services, it may not pay because of policy limits. Read your policy to learn more about limits.
Your insurance company may require payment for part of your services. Be prepared to make a co-payment when you check in for an appointment. You may be asked to pay for non-covered services in full before care is provided. For questions about copay, coinsurance, deductible or non-covered services, contact your insurance company.
Marshfield Clinic submits claims to primary insurance and, if applicable, secondary insurance, including Medicare and Medicaid. Patients receive a bill when an amount is due and payable, responsible for the amount in the “AMOUNT DUE” box. Balance due is payable when the statement is received.
For your convenience, payments can be made by cash, check or money order; credit cards (Visa, MasterCard, Discover, American Express); and online.
