Schedule appointments, or view details of past and upcoming appointments

Schedule your Medicare Annual Wellness Visit

After you have the App downloaded, click the link in your text/email message for direct access to schedule your appointment
Don’t have the link? Log into the app and select Appointments at the bottom

Select Schedule and hit Continue
Select Reason For Appt, search: Primary Care Physical/Well Adult and hit Continue
Select the location for your appointment and hit Continue
Review available appointments. Select the date, time and provider and hit Continue
Review your information and add any notes. Hit Schedule Appointment
