When your goal is to return home, the professional staff is with you each step of the way. The transitional care unit is located within Hillside Manor and connected to Marshfield Medical Center-Beaver Dam making medical imaging, lab and other medical visits convenient.
You will work with the interdisciplinary team at Hillside Manor to create a personalized treatment plan. Before you are discharged, a rehabilitation therapist will help you evaluate your home to ensure a safe transition and arrange needed equipment, home supportive services or outpatient therapy services. Our support continues after you are home, to ensure that you have correct medications, are receiving needed services and answers to your questions.
