Contact your primary care provider’s office to schedule your appointment.

• Call Monday-Friday, 8 a.m. to 5 p.m. to schedule a COVID-19 vaccine appointment. During all other times, please leave a message and we will contact you to schedule your appointment.
• A vaccine will be available to you at the time of your scheduled appointment. No need to wait in line or take a chance that a vaccine may not be available.
